It took me 4 months because I'm stupid. Guess what: many Surfers are likewise stupid. The idea of a good User Interface is to make it so intuitive that no Surfer would hesitate for even 10 seconds trying to guess what their next action should be. 10 seconds' hesitation = an excuse to click away from a webpage. That is why I'm so obsessive about seemingly minor UI details.
I show Live Chat with the sound disabaled (i.e., Volume = 0) because I don't want to distract the Surfer's attention from the rest of my webpage. That's why your friend was able to click the Volume button. Try the same test with the sound disabled. I guarantee that the first action will be to click the Enable/Disable icon. Then the Surfer will be perplexed (no sound yet). He may (or may not) figure out that "Volume" is a separate button requiring clicking.
Several ideas:
1) Place a "|" character between the Volume and the Enable/Disable icon
2) Change "Volume" to "Volume Adj"
3) Put a bounding box around "Volume" and a separate one around the Enable/Disable icon. Then put another bounding box around both buttons
